ASL-Is being amputated a speech impediment or an accent in sign language?

As far as I understand, sign language has signs with two hands. What happens if you don't have an arm? Or hand? Does this affect your ability to speak? Is this a dialect, accent or a speech impediment? What other things does this effect?
